Contact the state dental board. They should be able to direct you from there. All dentists are supposed to have their current practice location on file, but they will have his home address as well. You may have to go through a lawyer to get the money back. However, if the "business" closed and it was a corporation such as "John Doe, DDS, Inc.", he might be personally protected from litigation.
